Research on the use of mechanical vibration for pain management in pediatric needle-based medical procedures: A bibliometric analysis

Aim: This study aims to identify trends by conducting a bibliometric analysis of research on the use of mechanical vibration for non-pharmacological pain management during paediatric needle-based medical procedures.

Method: A literature search was conducted on 17 April 2025 in the Web of Science database with no publication year restrictions. The keywords used were 'pain', 'vibration', 'vibrator', 'mechanical vibration', 'child', 'children', 'infant', 'neonate', 'paediatric patient', and 'young'. The search yielded 229 studies, of which 220 were excluded due to not meeting the inclusion criteria, being ongoing, being duplicates, or lacking full-text access. VOSviewer software and Microsoft Office 2016 were used to analyse and visualise the bibliometric data of nine randomised controlled trials (RCTs). The following were evaluated: annual publication counts, citation counts, inter-study citation relationships, countries with the highest publication output and their citation counts, the most frequently used keywords, the study design, the sample group, the procedure type, the data-collection instrument, the mechanical vibration application protocol and the study results.

Results: Nine randomised controlled trials (RCTs) published between 2010 and 2024 were identified. The first three publications on the topic have each received 23 citations. The country with the highest number of studies is Türkiye (n = 5), while the United States of America has accrued the highest number of citations. The most frequently used keywords are “pain” and “vibration”. The most commonly studied procedure is the heel lance in neonates (n = 7). The most frequently used data collection instrument is the Neonatal Infant Pain Scale (NIPS) (n = 5). Seven randomised controlled trials (RCTs) found that pain scores in intervention groups receiving mechanical vibration were significantly lower than in control groups (p < 0.05).

Conclusion: In the majority of randomised controlled trials (RCTs) investigating the effectiveness of mechanical vibration as a non-pharmacological pain management strategy during paediatric medical procedures, this method was shown to reduce pain effectively. These findings suggest that mechanical vibration is an effective, readily implementable, non-pharmacological technique for reducing pain during common childhood procedures. Nurses may therefore wish to consider using it to reduce pain in paediatric patients during needle-based procedures.

Amaç: Bu çalışmanın amacı, pediatrik iğneli tıbbi işlemlerde nonfarmakolojik ağrı yönetimi için mekanik vibrasyon kullanımına ilişkin araştırmaları bibliyometrik bir analiz yoluyla inceleyerek eğilimleri belirlemektir.

Yöntem: Literatür taraması, 17 Nisan 2025 tarihinde Web of Sciences veri tabanında herhangi bir yıl sınırlaması yapılmadan gerçekleştirilmiştir. Aramada “pain”, “vibration”, “vibrator”, “mechanical vibration”, “child”, “children”, “infant”, “neonate”, “pediatric patient”, “young” anahtar kelimeleri kullanılmıştır. Tarama sonucunda toplam 229 araştırmaya ulaşılmış, ancak dahil edilme kriterlerine uymayan, devam eden, yinelenen ya da tam metin erişimi bulunmayan araştırmalar çıkarılmıştır (n=220). Toplam 9 randomize kontrollü çalışmanın (RKÇ) bibliyometrik veri analizi ve görselleştirmesi için VOSviewer paket programı ve Microsoft Office 2016 programı kullanılmıştır. Yıllara göre yayın sayısı, atıf sayısı, çalışmalar arasındaki atıf ilişkileri, en çok yayın yapılan ülkeler ve atıf sayıları, en sık kullanılan anahtar kelimeler, çalışmaların tipi, örneklem grubu, işlem tipi, veri toplama aracı, mekanik vibrasyon uygulama prosedürü ve çalışma sonuçları değerlendirilmiştir.

Bulgular: 2010–2024 yılları arasında yayınlanmış dokuz RKÇ çalışma tespit edilmiştir. Konu ile ilgili ilk üç yayın 23’er kez atıf almıştır. En çok yayın yapılan ülke Türkiye (n=5), en çok atıf yapılan ülke Amerika Birleşik Devletleri (ABD)’dir. Çalışmalarda en sıklıkla kullanılan anahtar kelimeler “pain” ve “vibration” kelimeleridir. Çalışmalarda en çok kullanılan işlem yenidoğanlarda topuk delme işlemidir (n=7). En sıklıkla kullanılan veri toplama aracı (n=5) Yenidoğan Bebek Ağrı Ölçeği (NIPS)’dir. Yedi RKÇ’de mekanik vibrasyon uygulanan girişim grubunun ağrı puanlarının kontrol grubundan daha düşük olduğu belirlenmiştir (p<0.05).

Sonuç: Pediatrik iğneli tıbbi işlemlerde nonfarmakolojik ağrı yönetiminde mekanik vibrasyon uygulamasının ağrıya etkisinin incelendiği RKÇ’ların çoğunluğunda (n=7) bu yöntemin ağrıyı azaltmada etkili olduğu kanıtlanmıştır. Bu bulgular, mekanik vibrasyonun çocukluk çağında sık karşılaşılan ağrılı tıbbi girişimlerde etkili ve kolay uygulanabilir bir nonfarmakolojik yöntem olduğunu göstermektedir. Hemşireler, pediatrik iğneli tıbbi işlemlere bağlı ağrıyı azaltmak için mekanik vibrasyonu kullanabilirler.
